All the Leave Request Forms must be submitted with the supporting documents to be considered for approval.
The ISE is a Dutch school and is therefore obliged to adhere to Dutch regulations. The Dutch Compulsory Education Act (leerplichtwet) requires parents to ensure that children attend school and the school and the city (gemeente) are required to monitor attendance. The ISE is only permitted to grant leave if a request for leave meets the strict criteria that is outlined in the regulations. You can find the full regulations HERE.
MEDICAL ALERT FORMS
The ISE has put together a medical alert sheet for those children at our school with any medical issues, for example, allergies or physical problems. Our objective is for staff and other parties to be aware of any medical issues, and to be able to respond adequately in case of a medical emergency.
On this medical alert sheet, parents can list the nature of the medical issue (diagnosis), treatment, and/or medication. We also ask for parents to list people we can contact in case of a medical emergency.
Please download the Medical Alert and Permission to Administer Medication forms and contact the school office at primary@isecampus.nl or secondary@isecampus.nl to submit them.
